This half-day learning opportunity and the toolkit were both developed by the Social Determinants of Health Reducing Poverty Sub-Aim Team of the larger Infant Mortality Collaborative Improvement and Innovation Network CoIIN-Illinois, which is led by the Illinois Department of Public Health’s Office of Women's Health and Family Services Division of Maternal, Child and Family Health Services.
